The base of our palliative-care service is NICE advice and stringent clinical governance. The plans all start with an overall assessment with regard to the physical symptoms, the psychosocial elements and personal priorities.
The provision of care is carried out by a team that is stable and has been carefully paired both in their areas of abilities and empathy to guarantee consistency and confidence. Families are well advised on their symptom monitoring, use of equipments and self-care and a 24/7 advice line is provided to reassure them on the spot.
Reviews of multidisciplinary nature are conducted on a regular basis to prevent the possibility of the occurrence of complications before they reach serious levels, the same ensuring that the number of the hospital visits are minimised and the amount of time at home/ work or social life activities maximised. Most importantly, we care about what is important to every person the ability to have meaningful moments, overcome the loss of dignity and make informed decisions at each step.
